In the film "Shutter Island" i couldn't understand what happened at the very end..?

No he was TOTALLY aware of himself. He was wondering if it would be better for him to have the lobotomy and forget everything he had done or stay as himself with the self-knowledge that his wife had murdered their children and he had then murdered his wife. He choose to forget.
